WHAT ARE MEDICAL SOCIAL SERVICES

Medical social services are an important and often underutilized part of home health care. While skilled nursing and therapy address the clinical needs of a patient, medical social services address the broader human needs that affect a patient’s ability to recover, remain safe at home, and maintain quality of life. A licensed medical social worker assesses the social, emotional, environmental, and practical factors that may be affecting the patient’s health and wellbeing — and then works with the patient, their family, and the rest of the care team to identify solutions, connect with resources, and create a more supportive path forward.
