Our Remote Patient Monitoring program helps support eligible Medicaid patients with structured home monitoring, regular follow-up, and meaningful communication that strengthens continuity of care between visits — all at no cost to the patient.
Many chronic conditions worsen slowly and quietly between office visits. High blood pressure, uncontrolled diabetes, and other ongoing health challenges do not always announce themselves until a problem has already escalated. By the time symptoms become obvious, they may already require urgent attention — or worse, a hospitalization that could have been prevented. Remote Patient Monitoring changes that. By tracking a patient’s health data at home and transmitting it directly to the care team, RPM creates better visibility between visits. It makes it easier to catch concerning trends earlier, respond more quickly, and keep patients healthier and out of the hospital. For patients, it means more support without more trips to the doctor. For physicians, it means more information and stronger continuity of care. For families, it means greater peace of mind.
Our RPM program is designed to be more than just a device. It is a structured, coordinated service built around practical home monitoring, organized follow-up, and clear communication between the patient’s home and their care team.
In practical terms, this program is currently centered primarily around blood pressure monitoring in the home. Eligible patients receive an easy-to-use blood pressure cuff, take readings at home as instructed, and those readings are transmitted electronically into the monitoring system. From there, readings are reviewed and followed up on according to the program workflow.
We work with patients, families, and physician offices to identify individuals who may benefit from structured home monitoring.
We review Medicaid participation, eligibility, and any applicable prior authorization requirements before enrollment begins.
Once approved, the patient is enrolled, consent is obtained, and they receive education on how the program works.
A cellular-connected blood pressure monitoring device is delivered directly to the patient's home. The patient is shown exactly how to use it — no wi-fi or technical experience required.
The patient takes readings at home as instructed. Data transmits automatically and securely into the monitoring system.
Incoming readings are reviewed and followed up on according to the program workflow. Any readings outside established parameters trigger escalation and physician communication.
Structured reporting keeps the physician informed throughout the program. Clinical data is shared with the ordering physician regularly.
This program may be appropriate for eligible Medicaid patients who have one or more of the following:
Our RPM program is built for real life — simple equipment, no wi-fi required, and clear instructions patients can actually follow. Every step keeps both the patient and physician informed, without adding extra work for either one.
Ordering physicians receive structured data reports showing trends, readings, and escalation activity collected between visits — giving them real insight beyond what a patient recalls in a 15-minute appointment. That visibility means earlier intervention, better-informed treatment decisions, and stronger continuity for your most complex patients. We handle the equipment, patient education, monitoring workflow, and all communication. Your office stays informed with zero added administrative work.
