What is Remote Patient Monitoring?

Remote Patient Monitoring — or RPM — is a structured health program that allows eligible patients to take certain health readings at home using simple monitoring equipment. Those readings are transmitted automatically to our clinical team, helping create better visibility into the patient’s health between office visits or home health encounters. At Tricare, our RPM program is currently centered around blood pressure monitoring. Eligible patients receive an easy-to-use blood pressure cuff at home, take readings as instructed, and those readings are reviewed and followed up on by our team according to the program workflow. No wi-fi is required — the device transmits automatically using cellular technology.

The process is straightforward and designed to be simple for the patient. First, we identify patients who may be appropriate for the program and verify their eligibility and Medicaid coverage. Once approved, the patient is enrolled and receives education on how the program works. A blood pressure monitoring device is then delivered directly to the patient’s home. The patient takes readings at home as instructed — typically at the same time each day. The device transmits those readings automatically without requiring the patient to do anything extra. Our team reviews incoming readings and follows up based on the program workflow. If a reading falls outside the expected range, escalation support and physician communication happen according to the care plan. The ordering physician receives regular reports throughout the program.
This program may be appropriate for eligible Medicaid patients who have one or more of the following: high blood pressure (hypertension), diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), multiple chronic conditions that benefit from closer monitoring, a recent hospitalization related to a chronic condition, or a need for more structured support and visibility between office visits. No. The blood pressure monitoring device used in our program is specifically chosen for ease of use. It is designed for patients who may not be comfortable with technology. The cuff is simple to apply, the reading process is straightforward, and the transmission happens automatically — there is nothing extra the patient needs to do to send their data. No smartphone, no app, and no wi-fi connection is required. When we enroll a patient in the program, we deliver the device to their home and walk them through exactly how to use it. We make sure the patient and their family feel completely comfortable before we leave.

No. RPM does not replace physician visits or home health services. It is designed to work alongside existing care — not instead of it. RPM fills the gaps between visits by providing structured monitoring and follow-up during the times when the patient would otherwise have no visibility into their health trends. Think of RPM as an additional layer of support. Your physician still manages your care. Your home health team still visits when ordered. RPM simply adds better monitoring and communication between those encounters so that nothing falls through the cracks.
Our RPM program is currently available to eligible Texas Medicaid beneficiaries. We currently participate with: Texas Medicaid (Traditional), Superior HealthPlan (Medicaid), Community First Health Plans (Medicaid), UnitedHealthcare Community Plan (Medicaid), Molina Healthcare of Texas (Medicaid), Wellpoint / Amerigroup Texas (Medicaid), and Blue Cross Blue Shield of Texas Medicaid where applicable. RPM participation, prior authorization requirements, and patient eligibility can vary by plan, condition, and individual circumstances. Please contact our office to verify current eligibility and program availability before enrollment begins. There is no cost to the patient for covered RPM services.

If a reading falls outside the expected range for your program, our team follows a structured escalation workflow. Depending on the reading and your care plan, this may include a follow-up call to you or your caregiver, communication with your ordering physician, and clinical guidance on next steps. RPM is not an emergency response system. If you are experiencing a medical emergency, always call 911 first. RPM is designed to help catch trends and concerning changes early — before they become emergencies.
Yes — and we encourage family involvement. If a patient needs assistance taking their readings, a family member or caregiver can help. During enrollment we provide education to both the patient and any caregivers who will be involved in supporting the program at home. The goal is to make the process as easy and consistent as possible for everyone involved.
