Streamlining the Referral Process

How we work with physicians, hospitals, and discharge planners to ensure smooth transitions and coordinated care for every patient.
At Tricare Home Health Services, we understand that for physicians, hospital discharge planners, and case managers, a smooth referral process isn’t just a convenience — it’s essential to patient safety and continuity of care.

That’s why we’ve built our intake and referral system to be as fast, transparent, and hassle-free as possible for every healthcare partner we work with.
Once a referral is submitted — whether by fax to 210-342-5030, by email to intake@tricaresa.com, or through a direct call to our referral line — our intake team begins processing immediately. We confirm receipt, verify insurance eligibility, and reach out to coordinate scheduling, all within a rapid turnaround time. We know that delays in home health can lead to unnecessary readmissions, and we take that responsibility seriously.

To initiate a referral, we typically require physician orders, face-to-face documentation, and the patient’s insurance information. Our team is experienced in working with Medicare, Medicaid, and most major commercial insurance plans, so we handle the authorization process efficiently and keep referring providers informed at every stage. If there are any documentation gaps, we proactively reach out to resolve them rather than placing the burden back on the referring office.

Throughout the patient’s care, we provide regular updates to the referring physician so they always have visibility into progress, changes in condition, and upcoming care milestones. Our goal is to serve as a true extension of your clinical team — a partner you can trust to carry out care orders with precision and compassion. For new referral partners in San Antonio and Bexar County, we welcome the opportunity to connect and explain how Tricare can support your patients after discharge.
