By tracking a patient’s health data at home and transmitting it directly to the care team, RPM creates better visibility between visits. What We Provide

How the Program Works

In practical terms, this program is currently centered primarily around blood pressure monitoring in the home. Eligible patients receive an easy-to-use blood pressure cuff, take readings at home as instructed, and those readings are transmitted electronically into the monitoring system. From there, readings are reviewed and followed up on according to the program workflow.

Identify Appropriate Patients

We work with patients, families, and physician offices to identify individuals who may benefit from structured home monitoring.

Verify Eligibility and Coverage

We review Medicaid participation, eligibility, and any applicable prior authorization requirements before enrollment begins.

Enroll the Patient

Once approved, the patient is enrolled, consent is obtained, and they receive education on how the program works.

Deliver and Set Up Equipment

A cellular-connected blood pressure monitoring device is delivered directly to the patient's home. The patient is shown exactly how to use it — no wi-fi or technical experience required.

Readings Taken at Home

The patient takes readings at home as instructed. Data transmits automatically and securely into the monitoring system.

Review and Follow-Up

Incoming readings are reviewed and followed up on according to the program workflow. Any readings outside established parameters trigger escalation and physician communication.

Ongoing Physician Support and Communication

Structured reporting keeps the physician informed throughout the program. Clinical data is shared with the ordering physician regularly.
